Browse for and attach files to the protocol in the Add Protocol Attachment section. For each attachment, a specific type must be specified. Required fields that must be completed for each attachment are Attachment Type, Status, File Name, and Description. The system will automatically generate the Uploaded By and Posted Timestamp fields.

Attachment Type | Required Field. Select the appropriate document type from the list of attachment types by using the drop-down menu and clicking on the appropriate list item to populate in the field. For the full list of Attachment Types and links to forms, see the Protocol Attachments section. |
Comments | Enter textual information about the attachment by clicking in the text box and entering text or clicking on the add note icon to view/edit/paste text in a new browser window and clicking continue to add the text to the protocol. Entries can be modified after the add action by typing within the text box and clicking the save button at the bottom of the page. |
Contact Name | To list the person to contact about each specific file, click within the text box and enter text into the field. |
Description | Required Field. To list the email address of the Contact Name, click within the text box and enter text into the field. Letters, numbers, and the @ character are accepted. | |
File Name | Required Field. To add the file name of the document selected to attach to the protocol, click browse to locate and select the file to attach. |
Phone | To enter the phone number of the Contact Name, click within the text box and enter text into the field. Only numbers will be accepted in this field. |
Posted Timestamp | The time and date the attachment was posted is set automatically by the system after clicking the add button. |
Status | Required Field. Use the drop-down menu to select Complete or Incomplete and indicate that the document being uploaded is in its final (complete) or draft (incomplete) status. All attached documents must be in Complete status in order for a protocol to be submitted. |
Uploaded By | Displays the User ID of the logged-in user who uploaded the attachment. |
The Attached Items section can be filtered to show only one specific type of document. Click on the drop-down menu and select the type of document you would like to see in the attached items subsection and click filter . Only the documents that have been indicated as the specific type will be shown. By clicking on the drop-down menu, documents can also be sorted by Attachment Type, Description, Last Updated, and Last Updated By. Filtered documents can be also sorted. To show all attached documents, click the “select” option in the drop down menu, which will return all attachment types associated with the protocol.
Once a protocol attachment has been expanded to display its details, shall allow the user to perform view, replace, and delete actions. The view will open the protocol attachment in a new window. The replace will allow the user to select another file to replace the existing protocol attachment. The delete action will trigger a warning message to the user asking to confirm the delete action and, once confirmed, it will remove the protocol attachment permanently from the protocol.
Protocols are versioned by the system anytime they are returned to the researcher and resubmitted to the IRB office. If a protocol attachment is replace during resubmission, the system will create a version of the attachment. Within the Attached Items area, the system will display the name of the user who last updated the attachment, the date the attachment was created, the date on which it was last modified, and a description for the attachment.
